High quality lugged frames are becoming popular again and there are many brands of lugs available.  For simplicities sake, I have decided to offer only two (although one of them has three different versions or styles).  Having decided that, I couldn't have picked two better brands than Richard Sachs and Pacenti.  Both are excellent quality.  The Richard Sachs lugs come in two different styles, the "Richie Issimo" and the "Newvex".  The "Richie Issimo" lugs are fairly traditional with a few twists.  The "Newvex" lugs are a modern take on the old European "Nervex" lugs.  They also come in a stainless steel version.   The Pacenti lugs are designed to be used "as is" or resculpted to look however you want them to (which is the reason for the open ended price).

These prices reflect the cost of upgrade from the basic lugged frame packages.

If you've made the decision to go with a custom fork with a cast crown, I think that you'll be happy with what I have to offer.  The Everest crown is a decent quality cast crown that also comes in an 1 1/8" version.  The Henry James crown is one of my personal favorites.  It is very high quality, light weight and beautiful.  The Richard Sachs "Newvex" crown matches the "Newvex" lugs and, like the lugs comes in a stainless steel version.   The Pacenti "Artisan" crown, also matches it's lugs.  Unlike like the lugs, it also comes in a stainless steel version.
